Over a billion of the world have hypertension, or high blood pressure, so that pretty much

means it's pretty common.

Let's start by defining it.

Typically, it's represented by two numbers: the top number is the systolic blood pressure,

which is the arterial pressure when the heart is contracting; and the lower number is the

diastolic blood pressure, which is the arterial pressure when the heart is relaxing or refilling.

Most of the time, blood pressure is taken in the brachial artery in your upper arm,

because if the pressure is high there, it's probably high throughout all of the arteries.

Normal systolic blood pressure is between 90 and 119 mmHg, and normal diastolic is between

60 and 79 mmHg.

Prehypertension, or on the high side of normal, is between 120 and 139 mmHg on the systolic

side, and between 80 and 89 mmHg on the diastolic side.

Stage I hypertension is between 140 and 159 mmHg on the systolic side, and between 90

and 99 mmHg on the diastolic side.

And finally, stage II hypertension is anything over 160 mmHg on the systolic side, and 100

mmHg on the diastolic side.

Typically, both systolic and diastolic pressures tend to climb or fall together, but that's

not always the case.

Sometimes, you can have systolic or diastolic hypertension, when one number is normal and

the other is really high.

This is referred to as isolated systolic hypertension or isolated diastolic hypertension.

High blood pressure is a serious problem for the blood vessels because it causes wear and

tear on the endothelial cells that line the inside of the blood vessels.

Just like a garden hose that's always under high pressure, in the long term, blood vessels

can develop tiny cracks and tears that can lead to serious problems, like myocardial

infarctions, aneurysms, and strokes.

Now, about 90% of the time, hypertension happens without a clearly identifiable underlying

reason.

We call this primary hypertension, or essential hypertension.

In other words, over time, pressure in the arteries begins to silently creep up.

And there are a bunch of risk-factors that we've identified for primary hypertension.

And these include: old-age, obesity, salt-heavy diets, and sedentary lifestyles.

With the exception of age, all of these can be improved with lifestyle changes.

And those changes can help reduce hypertension.

About 10% of the time, though, there is a specific, identifiable underlying condition

that is the cause of hypertension; and we call this secondary hypertension.

For example, anything that limits the blood flow to the kidneys, or the renal blood flow,

can cause hypertension, as well as things like atherosclerosis, vasculitis, or aortic

dissections.

This is because the kidneys play a super important role in blood pressure regulation.

When not enough blood flows to the kidneys, the kidney secretes the hormone renin, which

ultimately helps the kidneys retain more water.

That water contributes to more blood in the arteries, making them more full, which leads

to higher pressures.

Other diseases can also cause secondary hypertension.

Fibromuscular dysplasia, which affects young women, can cause the walls of the large- and

medium-sized arteries to thicken.

If it involves the renal artery and limits blood flowing to the kidneys, it triggers

more renin.

Another example is a tumor that produces excess aldosterone, which just like renin, leads

to fluid retention.

Finally, if the blood pressure becomes really high really fast, it's referred to as a

hypertensive crisis, and involves a systolic greater than 180 mmHg or a diastolic pressure

greater than 110 mmHg.

Hypertensive crisis can be further split into hypertensive urgency and hypertensive emergency.

With hypertensive urgency, there hasn't yet been damage to end organs like the brain,

kidneys, heart, and lungs.

In hypertensive emergency, there's shown to be evidence of damage to end organs.

So, for symptoms, usually primary hypertension isn't actually accompanied by any symptoms,

which is why it's sometimes referred to as a "silent killer".

Secondary hypertension may involve a variety of symptoms associated with the underlying

cause.

And finally, hypertensive emergency may involve symptoms like confusion, drowsiness, chest

pain, and breathlessness.

The first choice for treatment of hypertension is lifestyle changes, like changes to the

diet, exercise, and stress reduction techniques.

In addition, there are a variety of antihypertensive medications that may be given in some cases

as well.

Alright, as a quick recap, hypertension, or high blood pressure, affects over a billion

people around the world, and over time is a major risk factor for heart disease and

stroke.

Stage 1 hypertension is defined as 140-159 mmHG for the systolic blood pressure, over

90 - 99 mmHg for the diastolic pressure, and Stage 2 hypertension is defined as greater

than 160 mmHg on the systolic side and greater than 100 mmHg on the diastolic side.

Hypertension usually doesn't cause any symptoms, and the first line of treatment is lifestyle

changes.

7.14. Change How You Believe, Not What You Believe - Duration: 4:27.

In this series of videos, I've cast a lot of doubt on what we believe, and what we think we know.

I've argued that our systems of knowledge and beliefs are inevitably and inherently

riddled with omissions, oversimplifications, distortions, shortcuts, and flat-out inaccuracies.

I've argued that it's impossible for any ideology or institution to uniquely own or possess the truth.

I've argued that our sciences, our philosophies, our political ideologies, and our religions

can't be perfect, can't be as absolutely true as they claim, can only be partial contributors

to our big project of human inquiry.

But that does not mean I think you need to give up all your beliefs!

In fact, I think the exact opposite.

If you find value in believing what you do, if you see virtue and meaning in it,

if you find it to be a force for good—you should keep believing in it!

The main argument of this series of videos isn't that you should change what you believe.

Change how you believe.

Change what it means to believe.

We spend an awful lot of time and energy trying to change what people believe—and much of it is wasted.

Sure, white supremacists and other people with despicable beliefs need to change what they believe.

But do we really expect that we can convert all 7.5 billion people on this planet over to the same way of thinking?

Most people are good.

Most people are finding the good in what they believe, using those belief systems to become

a better person, and make the world a better place.

Where this goes off the rails is when we insist that billions of people have to believe exactly what we do.

The reason our sciences, philosophies, political ideologies, and religions insist on

absolute obedience isn't because they really have all the answers.

It's because they're participating in a great big social struggle for money, power,

authority, and followers—a struggle in which they have to present themselves as better

than all the rest, to get a piece of the pie.

So keep believing what you believe!

Who am I—or anyone else—to tell you what's most useful, meaningful, or valuable for you to believe?

If it's working for you, stick with it.

If it isn't, look at what else is out there.

See how you can broaden your horizons, and find new beliefs that will help you live your optimal life.

But don't fall for the seductive notion that one ideology has got it all figured out.

That you don't need any other belief systems to complement it.

Or that you can just give your brain over to one ideology, and stop being open-minded or humble.

Even if you believe in your religion, you can see the value in other religions, in science, and in other belief systems.

Even if your political beliefs largely fall on one side of the spectrum, that doesn't

mean your party really has all the answers.

Changing the how of believing is a lot easier than changing the what.

And it's the how—not the what—that makes our ideological struggles so hostile and deadly.
