Sawasdee kha. Welcome to Thai Veggie.

I'm Bie. Many of my recipes call for

vegetable stock. Today we are going to

make simple vegetable stock. You can

always use water but this vegetable stock

that we are going to make is guaranteed it's

going to be good. Here are our vegetables.

Now let's get started. What you are going

to use is a big pot like this. I fill it up with

water half-full. i just turned my heat

up to high and while I'm waiting for my

water to come to a boil

I'm going to prepare my vegetables. We are

going to use half of an onion. Just going

to chop it to big chunks like this

to make the stock so its going to cook in

easily. Now I'm going to add these onions

that I just cut into our stock.

I have here about six cloves of garlic.

I'm going to separate these garlic skins.

Put our garlic into our stock.

Just going to give it a little bit of

stirring here.

Next I'm going to add ginger. I'm only

going to use this much ginger.

Just going to slice it into small pieces

like this. And that's going to go in

the stock.

I have here about seven to eight small

pieces like this.

I'm going to add 2 stalks of celery.

I'm just going to cut it into big

chunks like this.

Now add it to our stock.

I have here parsley. I'm going to cut

the stems. These are part of the stem.

Just cut it to big pieces like this.

Here, into the stock.

A carrot. I'm going to use one big whole

carrot like this.

Cut into big chunks like this.

That's our carrot. And now it's our kale. These are

These are beautiful kale here. I have here only

the leaves because I took off the stems.

The stems of vegetables add

flavor to the stock and we use up

the whole vegetable.

No waste.

So, I cut all the stems.

I have here about 4 to 5 stems

of our kale.

Add that in here. My stock has come to a boil

boil now, so I'm going to turn down my heat.

Now I'm going to add soy sauce

and some black pepper as well.

Give a little bit of stirring here and

while we are waiting for our stock to come

to a boil, I'm going to cover up my stock.

Our vegetable stock is ready. I'm going

to turn off the heat and remove it from

heat. Now what I'm going to do next is

I'm going to let it cool off and once the

vegetable stock has cooled off, I'm going

to remove it to a container, so

that I can use it for the next recipes and

other dishes. As you can see my

broth has these big chunks of vegetables.

What I like to do sometimes is blend it

and can make a nice rich smoothie. I use

this broth to make my tom kha hed

and it works fantastic. OK, and

that is it for our vegetable stock.

It's very easy, very simple to make and

you can also use it

with other dishes.

Getting our thoughts together for the end of the year, the beginning of a new one, you know what I'm saying.

It was wavy.

We're here, we made it.

Uh, never thought I'd be alive this long.

I know that's weird to say, but like, I never in any serious sense thought past like, what

ended up being 22.

In 2016, my only verbal goal that I made to y'all here on the Internets was that I would

be more visionary.

[whisper echoes] Vision!

And honestly that is a goal that I will have to carry with me for the rest of my melanated days.

That is not something I can ever be done doing.

In 2017, I wanna maintain my vision, but in this video Ima share with you just some of

the ways in which, you know what I'm saying, some of the spaces and places my vision will

take me.

This is me claiming it.

[whoosh] I wanna go to Kenya.

I know I've said it so many times, it feels like.

And I'm not good at asking people for help.

So I wanna make content that pops, you know what I'm saying, that I can be proud of, uh,

visually as well as the content of the video.

And I realize that if I'm funny telling the story then that is in fact stand up I think?

But in my mind, it's different.

TV?

Documentaries.

I have a website, alright.

Haven't posted in two years.

That's because I realized I don't wanna be a blogger!

So I'm gonna revamp the website, do my lil, uh, photoshoot, alright.

My lil...do a little...you know what I'm saying, a little this.

And actually have a place where you can find all the different things I'm doing, you know

what I'm saying.

I always say see you on the Internet somewhere, and I really do be somewhere.

[whoosh] In 2017, and this is maybe the biggest one, I want to volunteer.

My election video, I don't want anybody...I feel like some people thought I was saying

that I don't care.

I care!!

As an Internet person, I know that I can reach more minds through, you know what I'm saying,

the interwebs, but in my community, what have I done.

That's all I'm saying.

Get in where I fit in, you know what I'm saying.

Um, and really be of service, you know I'm not nobody's spokesperson.

I'm just making sandwiches.

[whoosh] In 2017-- that's how you know I'm hype.

In 2017, I also challenge you to pour into people who need your love instead of wasting

energy on hate.

If you only have 24 hours in the day, is this how you really wanna use some of it.

Just a question.

I'm just asking.

I'm not saying ignoring the hate makes it die.

It's kind of like, uh, you know, being a Youtuber.

You will a thousand positive comments and then one comment that's like,

"Oooh, there's something in your nostril.

Yuck.

You ugly."

And it's like--- you gotta, you gotta come at them.

But you had all this love right here.

Pour into this love.

I just--for me right now, I feel like that's the better way.

[whoosh] Last but not least in 2017, I want to turn 27.

[whoosh] That's where my vision for 2017 will take

me I hope, uh, if I continue to work hard and have, you know, some forethought.

Okay?

I just wanna have fun, serve my people, mind my business, drink water.

And do the work that it takes to be louder than hate...to the people that matter.

504 Manifestation Determination Meeting - Duration: 18:00.

Hello, and welcome to the Accelify Webinar.

Here, we are going to be discussing and showing you how

to navigate to the 504 Manifestation Determination

Meeting and navigating through all your required forms.

Here, you can see the Global Home Page on the top left on

hovering over this navigation icon or waffle grid you will

see several icons that are available to you that will

help you easily navigate through the system.

Here, you will see the Home icon which will always take

you back to the Global Home Page,

the Calendar icon which will take you to the Calendar

section, the Settings icon which will allow you to edit

your Profile Information, the orange Plan icons which will

direct you to the Plan Home Pages as well as the Track icon.

On the right, you will see some features that you can explore.

You will see a Support icon which will show you the

Accelify Help Desk Phone Number as well as the Accelify

Help Desk email.

If you choose to email dadehelp@accelify.com,

please include your Employee ID,

your Student ID, the Event, Form and Field that you were

having issue or have questions about so that we can easily

investigate the issue.

I am moving down, you'll see the Miami-Dade County Public Schools,

your name and your role and that Logout action button.

Please note that you have to click on the Logout action

button to logout of the system.

Clicking on that 'X' icon on the top right of your screen

simply closes the window,

it doesn't log you out of the system.

Moving down the page, you'll see a blue bar that will

change according to where you are in the system.

You'll see those orange Plan icons as well as that Track

icon again.

Another way, you can easily navigate through the system.

As you move on, you will see important announcements from

Accelify, so please review these announcements before

moving on in the system.

Here, you will see several tabs in the Global Home Page.

You will see the My Student's tab where you will see all the

students you've added to the My Student's List.

In My Caseload section where you will see all students who

you've added to your Caseload.

In the My Calendar section, where you can view your calendar

and you can add or increase services and group services

and evaluations and you can add student activities and

activities to your calendar, so it's a great place to start

organizing your professional work week.

Here in My Services tab, you'll see services of your

students and service lines that have come over from

locked plans or service lines that have come over from the

previous system.

My Evaluations will show your Evaluations

for your students.

My Student Activities are non-billable services so that

will list all the non-billable services.

My Activities are activities such as Lunch Duty or Recess

Duty that you can also add to the Calendar as well.

My Groups, are groups of students that you servic so you can add

them in this tab and then also add the group service to your

calendar and the My Dashboard section which is a visual, okay it's a

pie graph of services that were delivered,

cancelled, scheduled or non-billable.

Okay, so here you can see the students on the

My Student's List.

I'm going to click into the 504 Plan that will take me to

the 504 Plan Home Page and here you see you can filter

through all students with 504, so you'll see the

announcements, you'll see here on the 504 tab.

You will see all of the events,

the upcoming events for students with a 504 plan.

You will see a link that will take you directly to that

event and you will see the ability to filter through all

the 504 students, the students with that 504 plan.

You can filter through this section by school,

as well as grade, et cetera.

Make sure to click on filter after you've inputted any of

these fields or you can click on Reset to clear these fields.

So, here I can search for My Student,

make sure that status is correct and select Filter.

Here I see my student appear, I'm going to click on that

student name.

To add the student to the My Student's List this was covered

in a previous Webinar you will go to that blue bar,

go to My List, My Students and then add your students there.

Here when we go into the Student Pages,

you will see that play button.

That play button will show a video that will take you

step-by-step through the process of navigating through

the forms.

You will see the Events section where we will create

our manifestation determination.

You will see that Student Information page which will be

information migrated over from ISIS,

Demographic, Disabilities, ESOL Information,

Medical Authorization and Contacts.

Here, you will see a Contact Log and Notes section when you

can add contact log and notes which pertain or do not

pertain to the plan.

Here are Attachments where you can view migrated attachments

from your old system.

You can also upload new attachments in this

section as well.

Here in the Assessment section,

you can create user-entered assessments you can do that by

clicking on that Create action button and you see that Video

icon is there for you to watch the video and here you can

create your 504 team as well.

Make sure to create your team here before you create your

notice so that you can then invite your team members.

So now we are going to go into the Event section.

Remember if you don't know how to create the team,

please click on that play button.

Here you can click on Create Event and choose

Manifestation Determination Meeting.

If you don't want to scroll down you could simply type

right inside that textbox, select the date you are going

to have the meeting and click on Save.

Here, you will see a Manifestation Determination

Meeting appearing in your Upcoming Events and you are

going to click on that Notice.

Remember, you first have to fill up the notice and then

distribute it in our system.

Here, you will see an Event Overview,

you'll see the left navigation mirrors the right navigation.

You will see color-coded circles next to each form.

These circles indicate that this form has either been

completed, is incomplete or is in the process of being

completed.

The red means that it's incomplete,

yellow will mean that it's in the process of being completed

and green means that it is completed and validated

and in compliance.

Here you will see that Student Information form,

which will provide you the information has been migrated

over from ISIS such as demographics,

disabilities, et cetera.

Here you will see the Distribution Manager where you will be

distributing that notice now we are ready to click into that

Notice of Meeting and fill out this form

and invite our team members.

You will see that Responses section which we will get to

in a minute.

Here, you will see the student name,

the school which you can select a different school or

you can click on other.

You will see red highlighted fields that means that those

fields are acquired.

You can either click on the Clock icon or the Calendar

icon or type right inside the textbox whichever you wish.

Here, you can type in your contacts for that parent if

they have any questions about the meeting and a good contact

number and also change the school if that contact is in a

different school,

and here you would select the purpose of the meeting.

Okay, so here I'm going to select 'Conduct a

Manifestation Determination'.

I'm going to scroll down and I'm going to invite my team

members using that down arrow and you can see those red

highlighted fields those are the required members that need

to be invited

and then I will validate and save my information.

You'll see grayed out areas that is for the parent to fill

out and when I validate the form and then save my

information and that information you know is

compliant and saved and

now we are ready to distribute the notice.

So I'm going to go in to Distribution Manager,

make sure that I see the parent,

check that the language is correct,

select the correct Method, going to mail this out.

I'm going to select both my Procedural Safeguards and

Notice of Meeting and click on Distribute.

This will form a distribution attempt on the bottom of my

screen so it will display that distribution attempt as well

as a PDF will appear on the bottom left if I'm in Chrome

and I can open this PDF file print it out and send it home

to the parent.

Okay, so that will include both those Procedural

Safeguards and Notice of Meeting.

Once that parent brings back that form signed and saying

they can come to meeting at that date and time you can go

back into the Notice of Meeting form.

Under Responses you can see that distribution attempt

click on the Pencil icon, select that Parent Response and Date

of Response and then select Save.

You can also click on the Paperclip icon,

if they handed you a signed Notice of Meeting document and

you can upload that signed form using the Paperclip icon.

To view this signed document, you can then click on the PDF

icon and print out a signed document.

So now, we are going to show a second Distribution Attempt.

These Distribution Attempts will appear on the front cover

of your Plan.

So let's say I want to have a follow-up phone call just to

ensure that parent can come to the meeting,

I will change my method to phone call,

click on that Notice of Meeting and then click on

Distribute and now I can go into that Notice of Meeting

and record that Parent's Response.

Remember, I'm going to click on the Pencil icon and record

the parent's response and the date of that response and then

click on save.

Okay, I do not want to lock the notice,

just yet, business rules recommend that you do not

lock the notice until the day of the meeting.

So you are going to use your breadcrumbs to go back to your

504 events.

So these are all links in the breadcrumbs to help you

navigate quickly in the system.

Now, I'm ready to start my 504 Manifestation Determination

so, I'm going to click on the

Manifestation Determination Event.

Here, I will see some forms that look familiar.

You see that Non-Discouragement form that's

going to need to be distributed.

Here I will see the Event Overview.

You will see those red asterisks for all those

required forms that need to be completed.

You see those color-coded circles that will tell you if

the form is incomplete, complete,

or in a progress of being completed.

You will see the Student Information form,

which will show you information of the student,

as well as the Distribution Manager where you can distribute

certain forms such as the Non-Discouragement form,

you can select that parent, select that Non-Discouragement

you can select any forms that you feel are necessary if you

want to redistribute the Procedural Safeguards or you

didn't distribute the Procedural Safeguards when you

were distributing your notice, you can do so here.

So here you can see that PDF is then downloaded to your

computer and you can print that out and have that ready

for you at the meeting.

So now let's say we're at the meeting itself,

we're taking attendance we are going to select if the parent

attended and we are going to see make sure that those

required participants have attended and then validate and

save your information.

Moving right along, we are then going to print this form

out and that will display all of the members that are

attending the meeting and they will have a space to sign.

For those members that you said did not attend,

they will not appear on the signature form.

So then later on, in the meeting, you can always scan and upload

this document.

You can actually lock the document and upload this after

you lock if that is easier for you.

Okay, so after the meeting, you can then scan this

signature page and then click on select files and then

upload that signature page.

Moving along you will see the Non-Discouragement form.

Here, you will see that Pencil icon after you make the

distribution and the parent response either they have or

have not been discouraged, and then you'll see the

Manifestation Determination form as you move on.

Here, you'll see those red highlighted fields that means

that those fields are required and you are going to input all

the necessary information, date of the Psycho-educational

reevaluation, the Date of the Most Recent R.T,

has a FAB been conducted, et cetera.

Has a BIP been developed, okay?

Is there a record of the behavior subject to

discipline, so, you're going to fill out all of these

forms, all these fields in this form as well as all the

forms that have the red asterisk.

Here you will see a textbox.

You can enlarge this textbox by dragging out on

the bottom right.

Here, you'll input any evaluation,

diagnostic results and information provided by the

parent, as well as observation of the student's behavior,

you are just going to put in some content just so that I

can show you how to navigate through the form.

Here you can see the current educational placement.

Based on a relevant information,

is the current educational placement appropriate?

You will select 'Yes' or 'No'-

'No', you will need to write an explanation.

Here in the second section, the Manifestation

Determination section you are going to record the behavior

in question on what it was caused by and if it had a

direct or substantial relationship to the

student's disability.

Okay, you're going to type in your explanation.

Or was the behavior in question a direct result of

the school District's failure to implement or develop an

appropriate IEP of 504 Plan and you will select 'Yes or

No' and then type in your explanation.

Here you can view the business protocol,

okay if the student's behavior was a manifestation of the

disability and the team shall and you should review these

sections and if the team determined that behavior was

not a manifestation of the disability,

then here is a paragraph of what the protocol is.

Then you are going to validate and save your information.

Here on the left hand side, you have the Procedural Safeguards

available to use as well as

the Parent Community Resource Letter.

You have a Contact Log and Notes sections so that Note

section will print out at the end of the plan so you can

create your Notes by clicking on Create.

You can also add any Contact Log/Notes by clicking on

Create here as well and filling out the necessary information.

Here, you'll see an Attachment section where you can upload

additional attachments.

So once I have had my meeting, I'm ready lock my notice and

the meeting itself and then go back into my Event section

make sure my event is compliant,

everyone signed, everyone is on the same page and then

whoever has permission will be able to lock this event.

So, if you have permission, you will then lock this event,

then the system will direct you back into the Event

section and the event will be in the Ended and Locked Event.

You will then lock your meeting,

and it will then show up in your Ended and

Locked Events section.

This will then become Read-Only.

You cannot go back and edit locked events.

If you wish to distribute any forms or any locked form,

you can go inside the event itself,

go to the Distribution Manager and then distribute any

finalized forms you wish to distribute.

So, you will have that available to you after locking.

So, only specific people are allowed to lock this event.

So, it really is up to your provider role that was given

How to Defeat Bad Habits - Duration: 2:42.

Habits, they make life easier.

They take out the thinking and leave you with the results, sadly, not all habits are good.

We can easily lose track of them and start fading into the depths of bad company, reaching

the point where they become comfortable.

Life may even seem impossible without them and it's that impossibility which makes

them difficult to break.

It's like we lost our keys.

We could stop, sit there on the porch and hope that they show up, but that's not very

effective.

We have to go out and make a new set, however, we can't go just anywhere.

We have to know where we're going and that's a difficult task if we don't even know where

we've been.

Start small.

Jumping in too deep is how most of us fail.

Pick one bad habit and record everything; when it's being done, the amount it's

being done, and for how long.

As an example, I occasionally fall into YouTube sprees.

It's when my motivation isn't too high and there was one point where I knew it was

bad, but I didn't know how bad it was, so I started tracking every minute and within

the first week alone, I watched seventeen and half HOURS of videos.

I could try justifying it, but there was no excuse.

I was spending too much time doing nothing and I needed to change.

Knowing how bad it really was allowed me to kick into high gear.

The next week comes and I cut back hard, not watching more than an hour each day.

Then I kept tracking; I had to stay on mark, but notice I didn't cut it out entirely.

That would've ended badly.

Initially, I could've stayed clean for some time, but once I get the taste back I'm

right back into the lost key stage.

I would've had to start all over again.

This is the problem; this is where most of us go.

I've been there and it always ends the same way.

If you cut it out entirely you relapse as quickly as you started.

Track, track everything of that one bad habit and only that one.

Eventually you'll realize you've learned a new good habit.

That's when you can move on to the next one.

It's difficult to stay patient, but we have to because small improvements are better than

no improvements at all.

It is I author and Internet entrepreneur, THE cerebral palsy vigilante Zachary Fenell.

Although today I am not really coming to you as the CP Vigilante.

After all, I am more than my disability.

Just like anybody else, people with disabilities are multi-dimensional.

It's not disability this, disability that 24/7.

We have various interests.

I shared one such interest of mine with the world when I published Rock Realities, a compilation

Now another interest of mine anyone who knows me can probably tell you.

Well, you too could probably figure out based off this video's title and what I look like

right now.

From head to toe I am a big Cleveland Indians fan.

So I wanted to give my thoughts on the team signing big free agent Edwin Encarnacion.

Admittedly when I first heard the Edwin rumors earlier in the off season I rolled my eyes.

I didn't think the Indians had any serious chance at securing his services.

Rather I kept my eyes fixated on Mike Napoli rumors, hoping the party at Napoli's would

continue in Cleveland.

Around the holidays when the un-confirmed word spread Edwin and the Indians agreed to

a three-year deal I felt a mix between shock and hesitancy, the big Nick Swisher and Michael

Bourn free agent signings prior to the 2013 season still fresh on my mind.

Between the original story breaking and the Cleveland Indians confirming the news however,

I have given the deal with Edwin more thought.

Thoughts I will share with you now.

Hence this video.

I've gone from shocked and hesitant to excited and hopeful.

Just consider the other names which come up when you talk about Edwin Encarnacion.

Sorry, I am still learning to pronounce his last name.

Names that pop up in Indians beat reporter Jordan Bastain's coverage of the signing like

Detroit's Miguel Cabrera and the Angels' Mike Trout.

That's the difference between hesitant and excited.

Edwin is an elite bat.

A bad year for him will still leave most Major leaguers envious of his stats.

As long as Edwin is healthy he will deliver.

And that's where manager Terry Francona comes into play.

Over the past four years in Cleveland Tito has shown an ability to manage older players

so they stay healthy.

Just look at last year's key acquisitions Rajai Davis and Mike Napoli.

Both put up some career numbers in 2016, an excellent year for Cleveland fans.

I don't think it is unreasonable to give Francona some credit for playing them in ways where

they excel without burning out.

Strictly based on stats Edwin Encarnacion appears a clear upgrade over Mike Napoli.

But what about team chemistry?

Everyone seemed to praise Napoli's clubhouse presence.

Will Edwin compliment the clubhouse culture or will he fit like wearing two left shoes?

Personally I felt reassured watching the press conference Thursday afternoon on the Cleveland

Indians' Facebook page.

Edwin mentioned he has previous ties with our all-star shortstop Francisco Lindor.

Despite only a year-and-a-half of big league experience Lindor has emerged as a locker

room leader.

Edwin and Francisco sharing a positive relationship is a good omen for Cleveland.

To sum up I am excited for Edwin Encarnacion to join the Cleveland Indians.

He is an elite hitter who on his bad days is good.

As long as he can stay healthy he should deliver.

Terry Francona's managing abilities should aid in that.

Add in Edwin's pre-established friendship with locker room leader Francisco Lindor and

qualms over team chemistry begin to ease.

Or at least that's my opinion.

Hello, and welcome to the Accelify Webinar.

In this webinar, we will be reviewing how to complete and

lock an IEP Re-Evaluation Team Meeting,

but before we get started and dive right into the meeting,

I would like to review the Global Home Page with you.

When you enter the system, you will see the Global Home Page.

On the far left, you will see a Navigation icon or waffle

grid, where if you hover over this you can see various icons

that will allow you to navigate quickly through this system.

You'll see the Home icon which will always bring you back to

the Global Home Page, the Calendar icon which will bring

you to the Calendar section, the Settings icon which will

allow you to edit your profile information,

The orange Plan icons which will direct you to the Plan

Home Pages and the TRACK icon.

On the right you will see an inbox where you may receive

emails from Accelify, a Live Chat feature which will be

available to you in a few weeks,

a Support Icon which when clicking on it,

you will see the Accelify Help Desk Phone Number as well as

the Accelify Help Desk Email.

If you choose to email at dadehelp@accelify.com,

please include your Employee ID, Student ID,

as well as the Event you're working in,

Form and Field that you are having questions about.

This will help us investigate the issue.

Here you will see a Settings icon,

which again will allow you to edit your profile information.

Below, you will see Miami-Dade County Public Schools,

your name, as well as your role.

Make sure you use the Logout action button to log out of

the system.

Simply clicking on the 'X' icon on the top right of your

screen will close the window, but it will not log you out of

the system.

As you move down the screen, you will see a blue bar that

will change according to where you are in the system.

Here you can see the My Students tab which will

display the students on your My Students List,

if you need help adding students to your My Students

List, please make sure you register for additional

The difference between the My Students and My Caseload tabs

is very important.

My Students are students who you are responsible for their

plan; My Caseload are students who you are responsible for

their services.

So make sure you're aware of those differences.

Here you have the My Calendar tab where you can access your

calendar, My Services where you can access your services,

My Evaluations, My Student Activities which are

non-billable services, My Activities, My Groups,

and My Dashboard.

For purposes of our webinar today,

we will be heading right into that Re-Evaluation Team

Meeting so that we can review what that looks like and how

you complete those forms.

So I'm going to click on My Students Name.

Once I click on that student's name,

I will be taken to the Student pages.

I will also see a question mark at the top of the page.

When clicking on this question mark,

a window will appear with a variety of training materials

available to you.

Here you can see the MDCPS training materials,

which includes the Quick Reference Guide that was given

to staff over the summer training.

You can also see the Frequently Asked Questions

document as well as a Workarounds document.

You can also register for additional webinars by

clicking on the Webinar Listing that will allow you to

access and view all those webinars that are available to

you and will allow you to register for them.

To access the Events section, you would click on Events

under the appropriate plan.

So here, I'm under the Miami IEP,

and I'm now going to click into the Events.

Here you will see your Re-Evaluation Team Meeting

under Current and Upcoming Events.

This will be populated for you,

and you will notice that after you lock an initial

eligibility meeting, then the Re-Evaluation Team Meeting,

will appear in your Upcoming Events with the appropriate

Due Date plus three years minus one day.

As you come closer to that Due Date,

you can always go into that event and start adding any

attachments or any documents that you feel will add to

that plan.

The Plan the Meeting can stay in the Upcoming Events section

in a draft form in an unlocked state as we call it in our

system for an indefinite amount of time.

So if you need to add to this document, as you see fit,

as you come closer and closer to the meeting,

then you can do so.

Just make sure you save your information.

So you do not have that 30-day limit or any limit to when you

have to complete this by after seeing this event in your

Upcoming Events section.

The data will not be lost.

Here you can see that under my IEP Re-Evaluation Team Meeting

Event Group, I have three events.

I have my Notice of Re-Evaluation Team Meeting,

I have my IEP Re-Evaluation Team Meeting,

and I have the IEP Re-Evaluation Meeting itself.

So let's first start with the Notice of the Re-Evaluation

Team Meeting.

Here you can see a white arrow at the top right of

every form.

When clicking on this white arrow you will see a video

that will appear on your screen.

This video will help you navigate through this form.

It will provide for you a step-by-step process on how to

fill out the form.

So if you have any further questions,

please make sure to access and view that video.

In the Notice of Re-Evaluation Team Meeting,

we are directed into the Event Overview section.

Here you will see the left navigation bar mirror the

right navigation.

You will also see on your screen red circles.

This means that these forms have not been completed,

they are incomplete.

As you start completing these forms,

these circles will turn yellow and when you complete and

validate these forms, these circles will turn green.

So if you have a lot of forms in a meeting or in an event

such as an Annual IEP Meeting, and you would like to view

which forms you've completed and which forms you still need

to complete, then make sure to click on the Event Overview.

Here in the Student Information section where you

will see the demographic information of the student,

disabilities, et cetera that are populated over from ISIS.

You will also see the Medical Authorization section as well as

ESOL information.

On the bottom you will see those Student Contacts.

Under Forms you will see Notice of Meeting.

You will notice that there is a red asterisk next to the

Notice of Meeting form.

This means that this form is required.

The red asterisk will not change color.

They will remain red.

That is okay in our system, it just means that these forms

are required.

Here you can see that white arrow in every form,

which is there to help you navigate through each and

every form.

When clicking on these white arrows a video will appear.

And it will help you navigate through the form.

As we scroll down the Notice of Meeting form,

you will see a Responses section,

which you will be filling out after you distribute the form

and I'll go into more detail about that in a minute.

Here you can see the Student Name,

you can see the School which you are able to change if

needed or you can select 'Other',

and type in the school.

Here you can fill out the date,

as well as the Start Time and End Time,

you can either click on the Calendar icon or type right

inside that textbox and the End time is an estimate.

Here you would type in the Contacts as to who the parent

would contact if they need further assistance,

so which faculty member, which staff member would be that

Parent Contact and as you can see,

as I'm typing in these red highlighted fields,

they are not highlighted in red anymore,

they are now white, which means that they are complete.

As I scroll down, I will see the Purpose of this Meeting

and here I will be selecting all the purposes of this

meeting, so I will say I'm conducting a Re-Evaluation

Team Meeting.

You will see that developing a statement of the Students

Transition Service Needs has automatically been checked off

since the student is older than 14 years old and this is a

Transition IEP.

As you scroll down, you will see that certain fields are

highlighted.

These are the required members that you need to invite so

after I create my team in that Student section in the Student

Pages, I can then select members of my team to invite

to the meeting and then I can validate my form which saves

it as well.

So once this form is validated,

I'm ready to distribute.

You can distribute this form by clicking into the

Distribution Manager.

Here I would select the parent making sure that the language

is correct and that I have the right method.

I'm going to select my form which is the Notice of

Meeting, and click on distribute.

After clicking on Distribute, I will see a PDF version of

that form appear on the bottom left of my screen where I can

click on it and then print it out for the parent.

I will also see a Distribution Attempt on the bottom of

my screen.

These will be the contacts printed out on the front page

of your Plan and when going back into the Notice of

Meeting, I can then record the response.

So let's say I gave this notice to the parent,

they signed, they said that they would come,

brought that notice back, I now can go into the

Notice of Meeting.

Under the Responses section I will see this distribution,

I will click on the Pencil to record that parent response,

and then click on Save.

I have to distribute at least two times.

So I'm going to do a follow-up phone call with the parent,

so that I can show I've made two contacts.

So I'm going to say Phone Call for method,

I'm going to click on that Notice of Meeting under Forms

and click on Distribute.

Once again, to record that response,

I would go back into the Notice of Meeting form,

click on the Pencil next to that recent distribution,

and record the parent's response and click on Save.

You can see that the Date of the Meeting and the Time of

the Meeting are populated for you.

Now, I'm going to use my breadcrumbs to navigate to

back to my IEP Events section.

I will not lock the Notice if I have not conducted

the meeting.

So if you plan to have the meeting last minute some other

day, you can easily go back into the Notice of Meeting

Form and change those dates and redistribute.

So now we are ready to go into the meeting.

Let's say it is the day of the meeting,

and we're ready to hold our meeting.

We are now entering the IEP Re-Evaluation Team

Meeting Event.

When I click into that event again I will see that

Event Overview.

You will see those red asterisks next to those

required forms.

You will also see red circles next to the forms that are

incomplete and once again you will see that white arrow

against that blue background in every single form.

Those are videos that will help you navigate through the form.

You will see the Student Information section as well as

the Distribution Manager

Now we're ready to take our Meeting Attendance.

Here we're going to select if the parent attended as well as

the student we are going to say if they Attended or Did Not

Attend and then click on validate and save.

And now we're going to click into our Re-Evaluation Team

Documentation form, where we are going to document the

program, the case, the date it opened, the date it closed.

We're going to document the dates of the Last

Re-Evaluation, as well as the areas that were assessed,

current levels of performance, and the informants.

Here you can see I can type into these textboxes.

You can fill out any necessary fields that apply to

this student.

Here in this section, we can fill out the required

assessment areas as well as the people responsible and the

date the report is due.

I'm going to click on the Date report is due.

As you scroll the bottom, you will see a very important

question, Is a formal assessment recommended?

Here you will select either Yes or No.

I'm going to select Yes and save my information.

Now I'm going to go into my Informed Notice of

Re-Evaluation and/or Consent to conduct a Re-Evaluation.

I'm going to see that there is a Responses section,

so this form would need to be distributed.

Here you can select the number that matches what you decided

if a Formal Re-Evaluation is needed or not.

I'm going to select 1 and say that a formal Re-Evaluation is

needed and then you can type in Factors which are relevant

to this proposal inside the textbox.

In the next textbox you will type in any of those special

programs that your child currently receives services

in, and as you scroll down, you can make several

selections here if the information provided to the

parent or guardian or other families,

or if the evaluation you know was data gathered since the

initial evaluation so you are going to select all the

information that you gathered.

So I'm going to make my selections and I'm going to

write my description as to what the parent/Guardian said,

et cetera.

I'm typing in all that information provided by the

teachers and professionals.

Here you can select a recommendation that is made

for formal assessments so you can select any of these areas

where the student needs to be assessed and underneath here,

you're going to write in those contacts for the parent should

that parent need to call someone if they have questions,

and who the parent should return the forms to.

You can then validate and save your information.

To distribute this form, click into the Distribution Manager.

Here you will select Parent, make sure that the language is

correct and that you select the correct method, the method in which

you choose to distribute.

Here you can select the forms which you wish to distribute

and then click on Distribute.

As you can see here, my Distribution Attempt shows at

the bottom of the screen, I also have that PDF version of

this form, which I can then print out

and hand to the parent.

Once that parent signs and gives consent,

I can then record this response in the system by

going back into the Informed Notice of Re-Evaluation and/or

Consent to Conduct a Re-Evaluation.

I will click on the Pencil icon and select that parent

Decision Yes, I agree, or No, I don't agree to a Formal

Re-Evaluation or No Re-Evaluation,

so No Re-Evaluation was recommended.

So Formal Re-Evaluation Recommended, Yes,

and clicking on Save.

Here I can then upload the signed document by clicking on

the Paperclip icon, and then I can view that signed document

by clicking on the PDF icon.

You can upload documents after locking the event.

The system does allow you to upload documents after you

lock events.

Once you complete the event, you make sure each of these

forms is validated by making sure you click on the

Validate Form action button and you can see that all your

forms are validated.

Now we're going to go back into our Events section,

where we will complete our Re-Evaluation Meeting.

So now I'm clicking into my IEP Re-Evaluation Meeting,

and as you can see here, I used my breadcrumbs to easily

navigate through the system.

These are all links that can take you to where you

need to go.

Here I have my Meeting Signatures form,

I can upload my form where those members signed.

I have Evaluation form where I can upload the evaluations,

I have the Mutual Exchange of Information form,

where I can fill out and also distribute to make sure the parent

consents to mutual exchange of information

then I save my information, and then go to the Distribution

Manager to distribute this form.

Make sure that you select your method,

and that the language is correct.

Remember you can choose Spanish or Haitian Creole here

if you need to.

Here you can select any of the forms in this section.

Here you will see Mutual Exchange of Information,

you can also distribute the Procedural Safeguards

here as well.

After you've distributed this form,

you will see that Distribution Attempt on the bottom of the

screen, as well as under the Responses section inside the

Mutual Exchange of Information form.

To record the parent response, simply click on the Pencil

icon, click on the Calendar, and record that date of response.

You can upload this signed form by clicking on the

Paperclip icon, and then you can view this signed form by

clicking on the PDF icon.

Once you have completed all areas and validated each form,

you can then go back and lock the notice and all the

subsequent events.

You do have to have permission to lock this event,

so if you do not have the right permissions,

you cannot lock this event.

So here I would go back into the notice,

and I would see that lock button at the very bottom I

would lock my notice first once I lock that notice the

system will bring me back to the Events page where I will

lock the Re-Eval Team Meeting and then the

Re-Evaluation Meeting itself.

I will then see these events in the Ended

and Locked Events section.

Once I lock these events, they will become Read-Only.

You will not be able to edit, add,

or modify any information inside these locked events.

So to reiterate, in an unlocked state, you can add,

edit, modify, do whatever you need to do,

you can keep an event in an unlocked state for infinite

amount of time, just make sure you save that information.

This allows you to really prepare for your meetings

ahead of time.

When an event is Ended or Locked, you cannot edit it.

You do not have any window or period where you can go back

and edit that information.

So before we complete the presentation part,

and I start answering questions,

I just want to reiterate a few things.

There is a question mark at the top of the page.

Please click on that question mark if you are having

difficulty navigating through the system.

When clicking on the question mark,

you will see a variety of training materials

available to you.

You will see that Quick Reference Guide that was given

to staff members during the summer trainings.

You will see a Frequently Asked Questions document,

so if you have a lot of questions,

please make sure to view this document since many of those

questions will most likely be answered in this document.

You can also view the Workarounds document,

so if you are having some issues navigating through the

system, there is a document that allows you to work around

it and keep on going through the meeting so that you can

complete that meeting and complete that event.

while the system is loading - and remember that the red

asterisks do not change color.

Those red asterisks mean that those forms are required.

Name, and then we clicked on Events,

but yes you can distribute the forms all at once.

How many forms do I need to fill out and print during the

RT Meeting?

The same amount that you needed before you used the

system, okay.

So whatever forms you used before the RT meeting,

you can access in our system as well.

So you can just use any forms you would like to access

before the meeting, you can distribute them and print them

out so that you are prepared for the meeting.

In terms of what you need to fill out exactly and content

those are definitely business questions,

but I can answer questions on navigating through the system

and where you would access a certain form.

504 Eligibility Meeting - Duration: 37:13.

Hello and welcome to the Accelify webinar.

In this webinar, you will learn how to complete and lock

a 504 eligibility meeting.

Here you see the global homepage.

On the top left you will see a navigation icon to click it looks like a

waffle grid.

So when hovering over this waffle grid,

you will see several icons that will allow you to easily

navigate through the system.

When clicking on the home icon you'll be navigated back to

the global homepage.

When clicking on the calendar icon,

you will be directed to the calendar section

of the system.

The settings icon will allow you to edit your profile

information.

The orange plan icon will direct to the plan homepage

and the track icon will bring you to the track section of

the system.

On the very right, you'll see several features that are

available to you.

You'll see the support icon and when clicking on the

support icon, you will see the accelify Accelify phone number as well

as the accelify Accelify help*desk email.

If you choose to email dadehelp@accelify.com,

please include your employee id,

the student id you're working with,

Your the event, form, field that you have questions about so

that we can better investigate the issue.

As you move down the page, you'll see Miami-Dade County

Public Schools.

You'll see your name, your role,

and the logout action button.

Please make sure to click on the logout action button to

logout of the system.

Clicking on the x icon on the top right will simply close

the window.

It will not log you out of the system.

Here you'll you will see a blue bar that will change according to

where you are in the system.

Here we are in accelitrack AcceliTRACK so we will components related to

accelitrack AcceliTRACK.

As you move down the page, you will see those orange plan

icons as well as the track icon and that is just another

way that you can easily navigate directly to that plan

homepage.

Below you'll see announcements,

okay please make sure that you read those announcements.

Those are important updates from accelitrack Accelify and here you'll

see several tabs.

You'll see my students tab which will list the students on

your my students list.

Those are the students who you are responsible for their plan.

In my caseload tab which will list students on your caseload

and you could add students in that section as well to your

caseload.

The my calendar tab which will display the calendar.

Okay you can organize and create services and schedule

services right on your calendar.

In The my services tab which will allow you to view all your

services, all those service lines.

You can create the service, a service line per for a student,

filter through your services and also export this page to a

CSV file.

In my evaluations, the same thing,

you can add the evaluation, create the evaluation,

filter through your evaluations and export it to a

CSV file, same applies with, the my student activities in

my activities tab.

My student activities tab or are your non-billable services,

my activities can include recess duty,

Launch lunch duty, if you're having a professional development one

day, you can include that and you can also include it right

inside your calendar to allow you to really organize your

professional work week.

In my groups, you can add groups of students who you'll you will

be servicing and you can schedule that group service on

your calendar.

In my dashboard, you can view a pie graph of scheduled

services, delivered services, cancelled services,

guys okay that's really nice visual for you.

When you click right on that color,

you actually will see which students have scheduled

services and how many.

Now we are going to go right into the 504 plan homepage.

We are going to click on the 504 orange icon and here you

can see that Miami 504 tab will be highlighted.

You'll see that the blue bar has changed according to

where we are in the system.

So here you see my lists and my students and we can add

students to our my students list here.

As we scroll down the page, you'll see important

announcements.

You will also see the students on the my,

on my students list who have 504s.

You'll see your their upcoming events,

the schedule date, the grade and location.

You will notice that in under event,

you will see links to that upcoming event.

So you don't need to click on the student name,

go to their events and then look for that upcoming event.

You have the link right there for you so you can quickly

navigate to that event.

As you scroll down, you also have the option of filtering

through all these students with 504 plans.

You will notice in the upcoming column,

you will have links to those upcoming events and in the due

column, you will see color-coded calendars,

green meaning that you have more than 30 days to complete

this event, yellow meaning that you have within 30 days,

you need to start completing this event okay?

And then red meaning that this event is 10 days away and you

need to start finishing these forms.

You need to start completing these forms.

So the number inside that calendar will indicate how

many days away you are from that due date.

So right now we are going to click on that student's name.

Okay here we'll be directed to the student pages so make sure

you are under Miami 504.

You will see the student information section where

information will be migrated over from ISIS.

You'll see the demographic information,

disabilities, dismissed exceptionalities,

ESOL level, medical authorization section as well

as contacts.

Here you have an event section where you'll see upcoming

events as well as locked events.

You'll be able to create events here as well.

Here in the contact log and notes section,

you have the ability to create and add contact log and notes

that may or may not pertain to that student's plan.

In the attachment section, you can upload

new attachments here.

You can also view migrated attachments that have come

over from your previous system.

In addition, you can filter through the migrated

attachments by clicking on the arrow and expanding the

window, typing in the textbox and then clicking on filter.

Here you'll you will have an assessments section too and where

you can create the assessment.

Okay so here you can create your assessment and save.

Then click on the down arrows to expand to see that score

and then click on that top arrow to collapse that window.

Okay?

Here you have the 504 team where you can create your team

so here we will do that.

We can click on create, select our participant,

remember this is just an example,

so we just created one of the users.

We're going to click on create you're going to see user,

the relationship and then you're going to click on select.

You can either find that staff member by first or last name,

or if you see that staff member there,

you can select, highlight that name and click on select and

then save, create, see say we need 504, 504

coordinator, okay.

So now we would like to also add the parent,

so we're going to click on the student contact tab,

click right inside that textbox and then click save

and then if you would like to add a custom user,

you could click on custom, if there's a new counselor,

so we're going to start typing in counselor if you don't want

to scroll and then type in her name.

You can also add the phone number and the email as well and

then click on save.

So there you go, we have our team.

Always create your team before you start billing filling out your

notice because when you're in your notice,

you will be inviting your team members.

So here we can see our 504 notice of eligibility meeting.

We're going to now click in to that meeting.

Here you'll see your event overview.

You'll also see your bread crumbs starting to form.

Okay, so here we know we're on in the right section of the

system.

Here in the event overview, you see color-coded circles,

red meaning that the form is incomplete,

yellow meaning that the form is in the process of

completion and green, which means the form is complete.

You'll also see that thing same, student information form which

displays, you know demographic information et*cetera that is

migrated over from ISIS.

Here you'll see a distribution manager form which I'll talk

about in a few minutes and here you'll see under forms,

that notice of meeting and the procedural safeguards.

Let's click into the notice of meeting form.

Here you'll see a white arrow on the top right of every form.

This will display a video that will help you navigate through

you're the form.

So I highly recommend that you click on this white arrow

to get that tutorial.

As we move through the notice of meeting form,

we'll see the responses section at the top where you

will be able to record parent responses after you distribute

in the distribution manager.

You'll see the student's name populated in this section.

Here you'll be able to select a different school if

necessary.

You can also click on other and

type in a different school there.

Here you can select the date of the meeting.

You can also, you can either use the icons then to select the

time and date or type right inside these fields and

whichever you choose to do, whichever is easier for you

and here you've got, you're going to provide some contacts

for the parent in case their parent has any questions.

Here you can see that the red highlighted fields which are

those required fields in our system.

They are clearing once I fill out those fields.

Now I'm going to discuss the results of a child's

evaluation or you can choose any other purpose that you've

see fit.

Alright here you're going to be inviting your team members

so this is just a dropdown and selecting those team members

that you've already created.

You can add additional team members as well but these are

the required team members that I need to invite.

How do I know they're required?

They are highlighted in red.

Here I can also add additional team members.

They you'll see some grayed out areas.

Okay that's for the parents to fill out.

Now this validate form action button,

it's highly recommended to select after you've complete

every form.

This not only saves your information but it validates it,

making sure that every required field is complete and

once we complete this notice, we will be ready to distribute

the form.

Remember, if the system is running a little slow,

just refresh and that usually solves,

okay so now we're going to go into the distribution manager.

Let's go fill that out one more time quickly.

We're going to fill this out

and validate is the right way to go.

Okay.

We're going to now select the purpose,

select our team members, make sure to save your information.

You can save and then validate but validate also saves,

okay?

So now, we're ready to go into our distribution manager and

distribute our notice.

Okay so make sure to add in your parent and I just clicked on

that student contact tab.

Here I'm going to make sure my language is correct,

the method in which I wish to distribute.

I'm going to say notification of U.S. mail.

I'm going to select the forms that I wish to distribute and

then click distribute.

This forms a distribution attempt on the bottom of my

screen, documents that I distributed.

You'll notice on the procedural safeguards.

I will see it here on a pdf file.

I will then be able to print out that pdf file of these two

forms and then you know, give that to the parent to sign.

Once that parent returns the document notice of meeting,

signed and saying she can come to the meeting,

it you will come into that notice of meeting again.

Click on the pencil icon and record that parent's response

and her date of response.

These are heard His or her, date of response.

Here you can upload that signed document by clicking on

the paperclip icon and then you can view that signed

document and print it out if you wish to by clicking on the

pdf icon.

So now, we would like to distribute one more time.

I've made a follow-up phone call.

So I'm going to change my method to phone call then

click on include, going to click on notice of meeting.

I'm distributing the notice of meeting information.

Here you can see my second distribution attempt.

So these distribution attempts will appear on the front cover

of your plan similar to what you,

similar to your contacts or parent contacts.

In our system, there's those contact log,

the notes, those are an added feature.

These are with the distribution attempts that

appear on the front cover of the plan.

Contact log notes, you can always document additional

contact logs and you can document notes.

That notes section will appear at the end of the plan.

Okay, so now that I've distributed the second time by

phone, I will go back into the notice of meeting form and

record the response.

I will click on the pencil icon and record that parent

response and then click on save.

Okay now, I have completed the entire event and in the notice of

meeting event, by then case my event is compliant but I will not lock

my notice until I have the meeting complete.

Okay?

Because there are any last minute changes or

cancellations or the parent wants to reschedule,

you could simply go back into the notice in an unlock state,

click on date, change the date and then redistribute.

But You do not have to create a new event.

So now they access the 504 initial evaluation.

I will go back to my 504 events page which I can easily

navigate to through my bread crumbs,

I'm going to click on my 504 initial evaluation.

Here I see my event overview with my color-coded circles.

That student information section,

I see my distribution manager again.

Here I have my parent consent to evaluate,

so I'm going to make sure I upload the parent consent to

evaluate form and record the parent response.

Parent agrees to evaluate and now I'm going to go through

and I can upload evaluation documents et cetera.

Private evaluation, here I have mutual exchange of

information and I can fill this out.

Use the save, remember validate form saves your

information as well as validates the form and form

has been validated successfully.

Here you can see that a response is needed so first I

have to distribute this document.

If I go into distribution manager.

Remember we can refresh.

Now we're going to be selecting our student contact.

Click on that checkbox, select add, make sure to include the

correct method and the correct language and then click on the

checkbox under include, click on mutual exchange of

information and then click on distribute.

Okay I can see a pdf file has formed in on the bottom

of to my screen.

I can open up this file and print it out.

Give it to the parent.

Once that parent has returned it,

I can then go back into the form,

click on the pencil icon and then select that date and then

click on save.

Okay, so now I'm ready to go, now I also have this the

procedural safeguards.

I can, I can also distribute here.

I have my contact log and notes section that I can also

access as well as my attachments that I can use if I

need to upload additional attachments.

Now I'm going to onto into my 504 events section and access

my 504 eligibility meeting.

Here I'll see my event overview which my,

with my color-coded circles, my student information and

distribution manager.

Okay I'm ready to take attendance.

We are starting the meeting and you'll see that the people

who you have invited had been populated into this section.

Here you can select if your team members have attended or

did not attend and then select validate form which will save

the information and validate it.

You can always add additional members here as well.

Now we are going to print this form out so this will appear

as a pdf file, you will print it out.

Those members will sign the paper.

After the meeting is held and everyone has signed and

agreed, you can even lock this event and then go back into

the meeting signature section and upload the signed form.

I see you have that meeting signatures form here in this

section.

Here you have the non-discouragement form which

you will need to distribute.

Okay, so you're going to need to distribute that.

You can do that ahead of time.

Here you have that 504 eligibility so you have a

parental input, you can choose your evaluation data,

Sign okay then you can save and validate and save your form.

Okay here you have your student section 504

eligibility determination.

Here you would select, does a student have a physical or

mental impairment?

You can say yes.

Create your disability, here there's does the physical or mental

impairment substantially limit one or more major life

activities.

It's to identify any major life activities or major

bodily functions by checking the appropriate box.

So you can select yes and make your choices,

select the priority and click on save and then you will see

that disability here.

In this section, question three,

does a student need section 504 services in order for his

or her educational needs can to be met as adequately as those

of non-disabled peers.

I'm going to say yes.

Here you have an analysis of the results of section 504

team's answers and then you're going to click on save to save

that information.

Here you'll have a summary.

You can select whatever is appropriate.

I'm going to select that we have determined,

there's a section 504 eligible and accommodation plan,

I can save and validate my form.

Here in the disability section,

I can then modify by clicking on the pencil icon and

inputting the appropriate dates and then clicking on save.

So here you go, here I see that,

you can even add my placement date,

oh, the placement date is for that initial so,

placement date is when you actually have that initial,

504 initial and then you can say that that is the placement

date for that student.

Here, 504 procedural safeguards is available to

you, contact log and notes, you can create contact log and

also create a notes and type in your notes here.

That will be printed out at the very end of the plan and

you can also expand the textbox by dragging out the textbox on

the bottom right, and then clicking on save.

Here you have an attachment section where you can upload

additional attachments.

And now I'm going to go into my 504 events and I'm ready to

start locking my events so I'm going to click on the notice.

I've checked my event compliance,

okay now I'm ready to lock.

It will become read-only.

After locking I'll be redirected back into the

events section and we'll see my eligibility notice here

under ended and locked events.

I can always print this out again,

distribute it, I can upload, scan and upload documents into

an event even though it is locked but I cannot modify or

edit information inside the locked event.

Now I will lock my initial evaluation.

Okay, if I validated every form and my event is compliant

then I can lock the event.

So here you go, you see that initial evaluation sitting

under ended and locked events.

Okay now in 504 eligibility meeting,

I believe the last thing we had to do is distribute that

non-discouragement form for this event to be compliant.

Okay we're going to refresh again.

Here I'm going to add that student contact and click on

add, make sure to select the correct method and that

checkbox include next to the right person,

so this is the parent, going to click on the

non-discouragement form and click on distribute.

You do have other options as well,

you know, as you can see here.

When you're complete, completely done and locked,

these, the eligibility documents in the eligibility

meeting, you can go ahead and distribute a draft or a final

if you want, you can distribute a draft once you

completed it and then you can lock and distribute the final

through the distribution manager as well.

It's your choice.

Here you have the pencil icon, right here you're going to say

have or have not been discouraged as parent.

Either has or has not been discouraged

so we distributed it,

we went right back into the non-discouragement form or we

recorded the response under the response section.

We can now upload this signed document by clicking on the

paperclip icon and view the signed document by clicking on

the pdf icon and now we're going to check our event

compliance.

Our event passed compliance successfully so we'll lock the

event, click on ok, and now a window appears.

Will the student's initial meeting be conducted today?

If I select yes, then I will not have to create,

we will not have to fill out another notice and they I will

just be able to continue and work in that initial meeting.

If I select no, then a new notice with initial meeting

underneath that notice will appear.

I'll select no to show you what that looks like.

So if you're having an initial meeting on a different day,

and now you'll see an upcoming event,

that notice of initial meeting and that initial meeting itself.

And then you can select the dates in that notice.

You'll also see a re-evaluation event coming up,

okay, so that will be three years minus today,

also populate okay?

So that concludes this portion of the webinar on how to

complete and lock the 504 eligibility meeting.

Would it be considered other as disabilities?

You would write if it was considered other in your

previous system, in other, it will be considered other here.

So, example,

click into re-evaluation and see what your options are.

No, no logical disorders including head injury,

I mean if you don't see that here,

then I would select other disabilities definitely and

then you could, you would have a place to record that.

Okay next question, how we can we get a list of the active

students on a 504 of our school?

Good question.

When you first enter the homepage,

you can go into 504, the 504 plan homepage or wherever you

want in the system actually.

You can use our, our navigation icon and click on

504 homepage and that will allow you to see all the

students with 504 plan.

You like to filter for a certain school,

students with 504 plans.

You could just select that school and then click on filter.

So obviously those kids, yeah, some,

you know, you can look for your school then change the

status which, trying to find a school,

students with 504, there's one.

Okay, so here I found a student.

They're active you can select active.

You can just select school, click filter and you'll see

all the students with the 504 plan in that school.

On, you know whatever due date you're looking for or whatever

upcoming event, and then click on filter,

and that would filter for students with only that

upcoming event or only that due event.

Next question, the 504 list of students on 504 students are

showing duplicate.

Sometimes if they have more than one plan,

they will show up I guess more than once or sometimes if they

have more than one upcoming 504 event,

they will show up more than once so for example,

looking all these events, so it really just allows you to

click on any of the event or the event that you're looking for.

Hello, and welcome to the Accelify Webinar.

We are going to learn about conducting an IEP Change in

Identification and Navigating to the Event section,

creating this Event and navigating through the

required forms in this event.

Here you can see we are on the Accelify Global Home Page.

On the top left, you will see a Navigation icon or waffle

grid and when hovering over this waffle grid you will see

several icons which will allow you to navigate easily through

the system.

You will see a Home icon which will take you the Global Home

Page, a Calendar icon which will take you to the Calendar

section, a Settings icon which will allow you to edit your

profile information, the orange Plan icons,

which will direct you to the Plan Home Pages as well as the

Track icon, which will take you the Track section.

On the very right, you will see features available to you.

You will see a Support icon which will display the phone

number for the Accelify Help Desk as well as the email

dadehelp@accelify.com.

If you would like to email, please include your Employee

ID, Student ID, Event, Form and Field that you have

questions about so that we can investigate the issue.

Below you will see Miami-Dade County Public School,

your role as well as your name,

your name roll and then the Logout action button.

If you would like to logout of their system,

please click on the logout action button.

The 'X' icon simply closes the window.

Moving down the page, you will see a blue bar that will

change according to where we are in the system.

You will see Announcements from Accelify.

Please review these announcements before moving on

in the system.

Here you will see several tabs.

You will see the My Students tab which will display

students you've added on the My Student List,

My Caseload which will display students we were who you are responsible

for servicing.

Remember My Students are the students that you are

responsible for their plan.

My Caseload Students who you are responsible for servicing,

My Calendar which will display a professional calendar,

you can add services, evaluations or activities to

your calendar, My services will display list of your

services, you can also add services here as well,

My Evaluations tab you can add evaluations here and see

current evaluations, Student Activities are non-billable

services.

My Activities are activities such as lunch or recess.

You can also add this right into your calendar.

My Groups, you can add a group of students you serviced and

then add it to your calendar.

My Dashboard is a pie graph of a visual of services that have

been scheduled, delivered, non-billable, scheduled

and cancelled.

So I am going back to My Students tab,

you will see a list of all the students you can also filter

through all students in the district like by clicking on the

checkbox and then for example, clicking on a certain school,

you can do also an Upcoming Event,

such as an Upcoming IEP and then clicking on filter and

then you get that information.

I'm going to reset, clear that filter.

So right now I want to take you to the IEP home page,

so I'm going to click on the orange IEP.

Here you can see a Question Mark appear on the top of your

page, this question mark will provide you with support

materials and training materials that were given to

the staff in the summer trainings.

You have a Workarounds document,

a Frequently Asked Questions document as well as the

Webinar Listing so you can register for future webinars.

Please make sure to check that out.

Here in the IEP Home Page you'll see the blue bar has

changed, you can add students to your Student List by going

to My List, My Students and adding your students there.

You can see the student on your Student List and their

schedule dates and their information.

You will also see links to that event that will take you

directly to that event and you can filter through students

with IEPs.

Here you can see that Upcoming Event is a link and it will

take you to the link so you don't have to click around,

this will take you right inside that meeting.

Here you will also see a color-coded calendar with the

number of days until this event is due.

So you see its red that means that that event is due within

10 days and it's your warning sign.

If it's yellow that then it's close to being due in those 60 days and

then if it's green, then you have a while to complete this

within you kind of start adding those forms or

assessments at your leisure and say that event is more

than 60 days until the due date.

So here we are going to click on a student,

and now we're under Miami IEP right we're in that

Student Information.

Here we're on Student Pages, where we will see our Student

Information form. and it will display our demographic

information, student's disability,

ESOL level, et cetera Medical Authorization and Contacts,

you will have your Event section where you can create

and view your events your Upcoming Events as well as

your Ended and Locked Events.

Here you can look and create your contact logs and notes

that pertain or do not pertain to the meeting.

You have an Attachment section where you can see your

migrated attachments.

This is from your previous system,

you can upload additional attachments here as well.

Here you have an Assessment section where you can add and

create a user-entered assessments.

There's Those National or State, have been added for you so you

can create a user-entered assessment.

I'll click on user entered and there's a video there for you

in case you need assistance in doing this.

Okay so there are going to be Play buttons and you can

access that video in every single form.

The + sign, you need to create a user-entered assessment,

baseline and target will be used for RTI purposes.

Okay we're going to click on that arrow to expand.

Click on that + sign to get the next tier and then we are

going to input the score.

It's very important you input that score.

Okay so we now have our Assessment.

Supporting you and your assessments at this level

before you get into the meeting because you only can

select Assessments that you've added in this level.

Also with the Team, you have to add your team,

you have to create your team before you can then create

your notice because you're inviting these team members.

So if there is a team member that you want to invite,

but you don't see in the dropdown and your Notice of

Meeting, you need to come to your IEP Team section in the

Student Pages and create that team member.

Here you can see you can edit, by clicking on the Pencil icon

and the 'X' which will remove that team member.

So now we're ready to create our Change in Identification

Meeting.

I'm going to make sure I'm in the Event section,

okay.

Event section under IEP, Miami IEP and I can see in my

breadcrumbs that I am in the right section.

If this area says EP, then I know I have to go back to the

student pages and go onto the right Plan,

okay, Miami IEP.

So here, I'll select create event,

okay, here I'll find change in ID,

I don't want to scroll, I can even type inside this textbox.

The system will find this event for me and I am going to

select my schedule date and then click on save.

Okay now I'm going to create my Notice.

So here you can see I have my Event Overview with

color-coded circles.

Red meaning that form is incomplete,

green meaning that form is complete,

and yellow meaning that form is in the process of

completion.

You can see the Student Information Page as well with

that demographic information as well as that additional

piece that I mentioned earlier.

Distribution Manager is here where we can distribute our

notice and here under forms you will find that Notice of

Meeting form.

I'm clicking into this Notice of Meeting form,

you will see a Responses section which we'll get back

to in a minute.

Okay, we'll be able to record our parents' responses here.

As we go down, we'll see the student name,

the location to the school which you can edit and also

select 'Other' and type in a different school if you wish.

You'll see red highlighted fields that will show which

fields are required.

You can click on the Clock icon or you can type right

inside the textbox.

Here you will be typing in those contacts that parent can

contact if they have any questions with the meeting or

you know where the meeting will be held,

what time, et cetera.

You can choose the location and then type in their phone

number with their contact number.

Here you'll select the Purpose of the Meeting,

so you can look through this these Purpose of Meetings then select

the one that is best for your meeting.

Also, if you don't see the Purpose for this Meeting,

you can always, you know, select Consider Revisions and

then type in what revisions.

But however, you know, this is,

here you can consider Student Placement and Exceptional

Education Center you have options here you just need to

look through those options.

Consider discontinuation of or dismissal from ESE,

et cetera.

Okay, so here are your options,

you can select those that apply.

Here you can select your team members that you are inviting

to the meeting.

There you see these red highlighted fields,

these are required team members that need to be

invited.

You will see a grayed-out section that's for the parents

to fill out and then you can validate the form.

The Validate Form action button saves your information

as well.

We now are going to go into the Distribution Manager and

we are going to distribute our notice.

Here you could see that there are Play buttons in every

forms so please click on the Play button so that you can

view the video.

We're going to select the parent,

make sure the language is correct and the method and

then you can select your Notice,

your Notice and Procedural Safeguards and then select

Distribute.

You will see that the Distribution Attempt at the

bottom of the screen.

These are the attempts that will be displayed on the front

of your Plan when you print it out that front page what used

to be called your Student Contacts or your contacts

rather.

Here is a PDF that appeared on your screen,

you can print that out for the parent.

Once that parent signs and returns it to the school,

you can then record their response.

So here, under the Notice of Meeting,

under the Responses section you now see this distribution.

You can click on the Pencil icon to record the parent

response and the date of their response,

and then save.

You can then upload this signed document by clicking on

this Paperclip icon and uploading that scanned

document.

So now I want to make my second distribution,

I'm going to go into my Distribution Manager,

distribute my Notice one more time,

let's say I want to make a follow-up phone call,

so I'll select parent, I'll select my method as a

follow-up phone call, I want it to stay the same in

English, the same language, select Notice of Meeting and

then distribute, and then you'll see that second

distribution at the bottom of the screen,

here under Distribution Attempts.

Okay, so now I go back into my Notice of Meeting and I will

record that response

and then save to save that information.

I will not lock this notice before having the meeting

itself just in case you have any last minute changes or

cancelations.

If you do, then you can always go back into the Notice of

Meeting form, change the date and redistribute in the

Distribution Manager. and You don't need to create a whole

new event.

Okay now I'm going to click into the IEP events in my

breadcrumbs, so the breadcrumbs allow me to really

identify where I am in the system,

make sure that I'm in the right place and it they will also

allow me to easily navigate back to where I want to go in

the system.

So if I want to go to the Student Pages,

I can simply click on that student name in the

breadcrumbs.

Now I am ready to start my Change in ID Meeting.

Okay so here you can see, I have my Event Overview with

those color-coded circles, they will display if my form

is complete, incomplete or in the progress of being

completed.

You will see red asterisks which will display the

required forms, okay.

So you need to complete these forms in order for this event

to be compliant.

Okay so let's start with our change in ID Data form and I

will skip to Meeting Attendance but I do want to

review this Data form before we move on.

Here you can say are you removing all ID

exceptionalities from the student in this IEP

change in ID?

Right now I'm going to say 'No' and then the date,

I just wanted to dismiss some, save the name of the person

dismissing, name of the person completing the change

in ID form.

Okay see I validated it, save my information.

This is important question to fill out so you need to fill

out this form before continuing.

Here we're moving on to IEP Meeting Attendance,

you're going to select if the student has attended or did

not attend.

Okay, or the parent, okay, so you can select the student as

well who did not attend or attended as well as other

required members and then we are going to save and validate

this form.

So in order to make sure that all those required fields are

completed, we validate and that form has been validated.

So now we are going to print out this form by clicking on

the Print Form action button.

This form will then download and it will display only the

members who have attended, because members who did not

attend you wouldn't need a signature for or they cannot

give a signature because they are not there in attendance.

So anyone who you said did not attend will not display on the

Meeting Signature Page.

Once you have passed around the page and everyone has

signed, you can then upload the signature page in the

Meeting Signature form, then select File,

and then select Meeting Signature form.

Here you can see that Signature form and print it

out again if you wish or remove it by clicking on the

'X' icon.

You can upload forms after locking.

It is important for you to be aware of,

so if you don't have time during the meeting or after

the meeting just to scan and upload forms which make sense

if you don't have time to do to scan and upload forms while

you're conducting a meeting.

You can definitely do so after you lock the event.

Here under Parent Consent to Evaluate,

you can upload your consent to evaluate form,

then record that parent's response,

you can click on the Pencil icon,

click on the parent's response 'Yes',

they consented it to evaluation.

Here you can upload any Evaluation documents,

okay you just click on select files and you'll be able to

upload documents.

Here in the exceptionalities form,

you will be able to create or dismiss or whatever,

this person looks like they, need exceptionality but you

are able to hear then consent, you can create any new

exceptionality or dismiss an exceptionality, et cetera.

So here you can see created, this little icon means

dismiss, okay I can say that this one is dismissed.

Okay, let's say now you need to create a different

exceptionality, you can click on that create action button,

select student is eligible, et cetera.

Select the current correct dates and then click on save to save

that information.

Here you have your IND Eligibility forms,

EDBD, SLD those are available on your Distribution Manager.

Okay so if you like to distribute those forms,

you could select the parent, select those forms that are

available to you and then click on distribute.

Here you will see the important form in this

section, the PWNPR, Prior Written Notice of Proposal or

Refusal that's what was your old FAPE form.

It's the same form just a different name.

Here you are going to select those contacts and phone

numbers for that parent to contacts if they have any

questions.

Here you'll see this populated based upon an IEP change in ID

held on this date, this is what is being proposed for the

child, the Change in Identification.

I am going to select the student continues to be

eligible for and is now eligible for and now I'm going

to meet make my selection and say he is no longer eligible for

and say that I dismissed this one,

here I'm going to type in my explanations. This student was

found, okay.

Here you can add new records as well and options considered

and then why was it it was rejected.

If you wish to save these then click on save changes,

if you wish to add a new record,

click on add new record and if you wish to cancel the

changes, then click on cancel changes.

To remove this record, just click on that 'X' icon.

Here you can input evaluation procedures,

tests, records or reports used as a basis for the proposed or

refused action.

Remember you can always drag out the bottom right of this

textbox to expand.

Okay, and here if you need to make any other selections you

can do so, evaluation, change in educational placement,

provision of FAPE, et cetera and these areas look similar.

And then you can validate and save your information.

Here you can see the form has been validated successfully.

As you scroll down, you will also see Procedural Safeguards

are available for you if you did not distribute them in

your notice.

You can see a Contact Log and Notes Section,

so these notes will be printing out at the end of

the meeting.

Here you can create notes by clicking on create and type in

the comments.

And here you have an Attachment section available

to you to add additional attachments.

Okay, so now I'm ready to Lock my Notice and Lock my Event.

I just want to reiterate that question mark on the top of

the page as well as those Play buttons in every form that

will help you navigate through the system and really take you

through step-by-step on how to complete the form.

So now I notice that my Notice of Meeting here has passed

compliance, I will go ahead and lock this event.

Once I lock this event I will be redirected into the Events

Pages where I will see my Change in ID Meeting waiting

for me in my Upcoming Events.

Now I will click on it and I check compliance,

it was okay, I will lock it.

Okay so here we can see that at least one group is required

in the PWNPR under the FAPE section.

So the system will not allow you to lock if the plan is not

in compliance.

Okay, so, that was a validation error it told me

what I needed to complete so I knew exactly where I needed to

complete the information in order for this to be in

compliance.

And right now I'm just putting in some content so that it can

show you what this will look like.

But obviously you will be putting in content that

pertains to the student.

Here again we will be filling up this text boxes make sure

to save those changes and then other factors,

and then validating and saving that information.

Now we're going to check Event Compliance again,

and now we are going to our Event passed compliance successfully,

we are ready to lock this event,

it will become Read-Only I cannot go back and edit so

it's important for you to know,

and here you see in the ended and locked events,

my Change in ID Meeting.

If you wish to distribute any of these finalized forms,

finalized documents you can click into that Change in ID,

go to Distribution Manager, okay then you will select your

recipient under the include section.

Make sure your method and language are correct and then

select any of these forms which you wish to distribute.
